The Visiting Nurse Case Manager, Behavioral Health/Dementia Health, ensures quality and safe delivery of  comprehensive short-term intermittent nursing and home health services. Provides evaluation and assessments, acute psychotherapeutic and/or supportive interventions for patients and caregivers, and linkage to the community to maximize their level of functioning.

JOB DUTIES:

Evaluation and assessment using evidenced based Assessment Tools for Behavioral and Dementia care.

Develops, implements, evaluates  plans of care for patients and when appropriate caregivers.  Addresses problems with plan of care, attends and initiates client case conferences, provides coordination of the plan of care and maintains continuity of care with other health professionals assigned to the case and communicates recommendations for additional home health care services to the appropriate individual in a timely manner.

Provides education, supervision, and counseling to the adults regarding  home nursing procedures and other care needs as appropriate to client's needs. 

Documents timely and accurate computer entry of patient/job information to facilitate the development of the OASIS data set, orders for service, creation of time sheets and other required patient/employment documents and reports.
