Schedule: As soon as possible - ongoing, 24-hour call

Types of Cases: Surgery Experience and Endoscopic vein harvesting (EVH) experience for coronary artery bypass graft (CABG)
Credentialing Timeframe: 4 weeks
Electronic Medical Record (EMR): Epic
Must have an active Illinois license
